Guidelines for Claim Forms submission for Medical reimbursement
For reimbursement for lenses, please provide:
1. Ophthalmologist’s (or Eye specialist’s) prescription copy
2. Lens rate only (or) Frame with lens (with breakup details of lens)
3. Self-declaration certificate: I have not purchased a lens in the last 3 years.
